First off, bee not only delivered a fatal blow to sasuke twice, but Bee didn't die. He escaped on his own terms.

He may not be genius but he certainly is not an idiot, Tobi who is arguably the smartest ninja in Narutoverse said that if Sasuke see's an opportunity he uses it + he already knows Bee's fighting style and besides Sasuke could spam Enton covered Susanno arrows while Bee is trying to capture him.

As for holding back, besides amataratsu, Sasuke has nothing that can actually kill bee. Bee is a much better swordsmen and counters lightening.
Save Karin? she helped him (and jugo) more then they hindered
