Both are Top Notch Ninja, but Nagato has more knowledge of Itachi's capabilities throughout the manga than Itachi has on his capabilities, which should give him the advantage. Also Nagato is a poor match up for Itachi considering Uchiha are better 1v1, than against multiple enemies, so Nagato's Pain Rikudo technique making it 1v6 or 1v7 will lower Itachi's effectiveness. So I'd bet on Nagato to win due to these 2 things.

Itachi only sealed him because he had help with B and Naruto other than that, if itachi uses susanoo, chibaku tensei crushes it, if itachi uses amerterasu, and it spreads, then pain/nagato jumps in the air, and almighty pushes it everywhere, which means its either hitting itachi, or the amerterasu will hit him, nagato wins this low-mid difficulty
