Gaara because Tsunade is a close range fighter, and she doesn't have the speed to close the distance so her strength doesn't mean jack. Her only chance is to use Katsuyus acid to deteriorate the sand.

Garra his being show to be extremely strong but then again Tsunade is from the Senju clan but we havent seeing much from her Strengh,Medical Ninjutsu and Summoning

Strengh well 1 hit and garra is done for but garra never let anyone hit him Garra thinking ( if they get to hit me a done for ) so no he wont let himself get hit

Medical Her Special Release jutsu wont work since she will be crush into nothing so i dont thing it will work

summoning slug it slow and poison wont get near him his long range he wont let himself be caught by it

Well in defense of Tsunade she is a Senju and an Uzumaki (presumed) So her life force is very strong as is her body. She is also an expert in evasion and might have some long range up her sleeves as a result of her evasion tactic. So it might be a tougher fight then most people would think.
However the end result I'm sure would be the same Gaara wins. His sand is long range and very fast hell all he has to do is stay high above the ground shielded and Tsunade shouldn't stand a chance.
But Tsunade shouldn't be underestimated though she does seem like she sucks she is actually very skilled they just don't portray that very well in the series.

I think we can safely assume that she is part uzumaki as her grandmother Mito was an Uzumaki unless of course she ends up to be illegitimate or adopted. Tsunade is supposed to be kind of a legend which we haven't seen so far (as hokage she was pathetic in the invasion by pain) but she is a sannin, experienced and should not be taken for granted. it seems kuniochi are a bit weaker than their male counterparts. The story has been a bit sexist in that respect.
