I agree with Lichcraze,
Tsunade has incredible strength and she knows her way around a fight. She also knows incredibly many healing powers which probably could not get in use though. But Tsunade is quite quick actually and we havent realy seen everything she can so id say Tsunade would win.

Creation Rebirth? Who knows.
Hidan could potentially kill almost anyone unless they knew of his abilities.
Tsunade can counter Hidan's ritual with Creation Rebirth. He sticks it in her heart, she can recover before she dies, the recovery is almost instant. Afterwards she can drag him out of the circle to continue the battle. Besides, Tsunade will make sure not to get her blood spilled because she has knowledge of her enemy. Another thing she could do is summon Katsuyu, then Hidan has lower chances of getting a hit on her. Also, she can always get close to him and, regardless if hit with Hidan's scythe or not, she can rapidly use her Lighting Release to paralyze him completely. This way he cannot go into the ritual and she obviously wins the battle.
Tsunade should not get close-ranged with him unless it is a sure shot. Her chances are mainly with Katsuyu, relying on Creation Rebirth for help. Besides, Hidan is stupid..I believe Tsunade's intelligence will put her at an advantage.

Hidan taijutsu > Tsunades Taijutsu.
The dumbest thing I've read today.Hidan taijutsu consists of wild sword swinging, which can be easily countered.Tsunade can literally change the landscape with a flick of her finger.Just to compare, look at the damage Sakura did to the surrounding areas when she and naruto fought kakashi, or when sakura took on Sasori;Well sakura strength is like that of a baby compared to Tsunade's.You want to know how she can counter Hidan's weapon,she can used a similar strategy to shikimaru, except she doesn't need to prepare a hole in the grow, she can easily make a crater for hidan to fall in while he's attack.And with Hidan's lack of intelligence, it would be child's play for her.

okay smartass lets see if hidans taijutsu is so bad how did he keep up with kakashi in their fight?
and tsunade and sakura are too slow when they try to hit someone
Don't forget Tsunade's especially trained in dodging/avoiding hits. She stressed the importance of this to Sakura during her training; a medical ninja cannot get hit in battle - dodge dodge dodge.
When it all comes down to it, it really is whoever can get in the first hit. If Hidan gets the first hit then his ritual seals the deal. If Tsunade gets the first hit then Hidan won't have a functioning body. Even if he can't die it doesn't mean he can regenerate shattered bones.

Edit: Even Tsunade's Genesis of Rebirth wouldn't work. Once Hidan initiated his ritual any damage done to him is done to Tsunade. Even if Tsunade heals herself over and over again it would be useless. She wouldn't be able to fight back because her monstrous hits would only damage her that much more.

If tsunade is able to drag hidan out of the cursed circle, which she definitely can, then she would be able to beat him when she is healed.
It's not exactly a matter of "who gets hit first", because assuming Tsunade gets hit frist (unlikely) she can at that moment, or rapidly afterwards get in range to paralyze with her lighting release. Stopping the ritual and all further movement from Hidan.

With creation rebirth, she would heal all injuries instantly. In away she gains a type of immortality where she will heal all damage done to her. However, it will only last as long as her chakra stored in the seal lasts and I think that it would be sufficient time in order for Tsunade to beat Hidan.
