She can only use it once (that's all it showed her use it) because as she stated to Orochimaru she has chakra stored in her for head and used it to reproduce her cells faster to heal herself. If tsunade is severely injuried and uses it (against Orochimaru) then she'll used to much chakra and turn into an old lady. She can't spam it in a battle just use it wisely

It seems like most of the people that have voted just go right out and think "Oh, Tsunade can only heal, she sucks, Kakashi OBVIOUSLY"..No.
In my opinion Tsunade is a very powerful Kage. If Tsunade uses her Yin Seal: Release, to then proceed and activate her Creation Rebirth technique or if she summons Katsuyu, it's unlikely for Kakashi to win. By using massive amounts of chakra available after using the Yin Seal: Release, the Creation Rebirth Technique is able to continuously regenerate the cells in her body, healing any wound and even growing missing organs. If that's not enough, she can summon Katsuyu. This can serve for two purposes, offensive and defensive. She can either make it a giant and use Katsuyu's Tongue Tooth Sticky Acid technique or let it remain "palm-sized" so it can heal her in battle. If this has not raised doubt upon Kakashi's potential in winning this match, maybe the fact that she has an incredible amount of strength might do it. One good hit and it is good bye Kakashi. Tsunade is also a master at Ninjutsu and Taijustsu. Adding to all of this, she has Lighting Release jutsus that can paralyze Kakashi or even out the battlefield if he decides to use his lighting style techniques. She also has amazing stamina and knows a little Genjutsu. IMO Tsunade wins. Kakashi has his good abilities...but they are either hard to land a hit or to tiresome to do continuously.

Don't forget Jiraiya was the 1st choice for the 5th Hokage and Kakashi wasn't at his current level as he is now when tsunade was made the Hokage. Great example is when Kakashi fought against Sakura and Naruto, kakashi said Sakura's hit then he's done for but it doesnt matter if she can't hit him (she couldnt). Kakashi is fast enough to keep up with Itachi in their 2nd fight (something he couldn't do in the 1st fight 2 1/2 years before).

Note Kakashi is a swag beast, but maybe DESTROYED by Tsunade if one power packed punch is delivered directly towards him. people rant on about how kamui will be a key factor in kakashi's victory, yet no one is taking into account tsunade's monstrous power which ACTUALLY HAS ACCURACY COMPARED TO KAMUI. kamui is a joke, powerful but has little accuracy and uses 1/3 kakashi's chakra. fanboys argue that kakashi's chakra has risen to a point where he can use 6 kamuis, what ludicrous rubbish that I am hearing. kakashi amitted that had he used kamui against kakuzu he would have been in the hospital bed, note, this was not too long ago during the kakuzu hidan arc that if kakashi used one meesly kamui he would have been hospitalized, what to say had he used 6 kamuis like some fanboys claim.
lightning blade and raikiri will be virtually effectless, tsunade tanked two stabs from the kusangi blade and healed like nothing, im sure she can be impaled by lughtning blade without being killed right away.

I personally believe tsunade would win. Orochimaru stated that 1 punch from tsunade = instant death. Also she has the giant slug as a defensive tool and she has her regeneration technique for if she is in trouble. If kakashi does land a raikiri on her she can just do either the regeneration or have the slug heal her instantly. Either that or the slug can protect her and she can do a fatal hit on kakashi. I really dont see kakashi beating her. She has too much defensive healing and too fatal of an offense for kakashi to be able to fight her. Regardless i really dont care who would win. because they will never fight. my opinion is tsunade. i dont see kakashi doing anything her regeneration, slug, or she can instantly heal and counter.
TBH the only reason why oro survived against tsunade's hit is because oro is a healing beast who regenerates like crazy. he tanked bijuu hits so the fact that punches from tsunade almost killed him speaks volumes of her monstrous strength

Medical Ninjutsu is extremely useful. Yin Seal: Release = Creation Rebirth = You must either cut her in half or cut her head off, if not she will recover. She can basically own you with her expertise in Taijustsu or with her Lighting Release attacks (which paralyze you completely) while she's in an "Immortal State" or wait for you to run out of chakra. While if Kakashi tries to do a Kamui, he'll miss and just end up in the hospital. Kakashi has low chakra reserves and low stamina. Plus kamui is so inaccurate.
Btw, I wrote a whole paragraph explaining why her skills would give her the win a little while back.

Tsnuade may not be fast but she has way better stamina then Kakashi who is one fart away from overspending half his chakra. Mind you in her fight against Kabuto, he had to take a soldier medical pill to hang with her physically. Tsnuade has all the means to beat Kakashi

Only thing Tsunade has is chakra. And Kakashi can beat that with all his skills^^.
