Well, I had this theory that when Madara left the village, he found out Tobi, a person from another clan who also hated the Senju.
We know back then, clans were widespread, and villages were just forming. And Tobi being from another clan isn't highly ridiculous - it could explain his unique Space/Time jutsus. Plus, if he implants a Sharingan, it'd always be activated like we always see.
So Madara tells Tobi a rough outline of his Moon's Eye and Tobi becomes his most loyal follower. When Madara dies, Tobi is to "take over" for him, adopting his identity and continuing with the Moon's Eye plan. When the Moon's Eye is complete, Tobi would have Nagato use Rinne Tensei to revive Madara, so that he can take over as Supreme Overlord
Tobi claims to be Madara (on the latter's orders) so that people will fear the idea of Madara - as Tsunade stated in the previous chapter. Long theory and there are bits and pieces unexplained so that's why I never put it up xD
