This is the Itachi before his fight with Sasuke, so his eyes are kind of healthy.
This is the Naruto after his fight with Pein, but he hasn't achieved KM yet.
This is the SPOP after their fight with Jiraiya.

MS Itachi is going to only fight for a short while before his disease kicks it, and let's not forget about his eyes blurring up.
