kekkai tota combines three elements, not just two like a kekkai genkai. It also appears that it can be tought, unlike kekkai genkai's that appear to be passed down through bloodlines.

Kekkai Tota means bloodline expansion. Kakkai Genkai means bloodline limit.

Yes they can, which is the fact that brings the entire "bloodline" part into question.

I personally think kishi painted himself into a corner by putting Ice in the same category as Sharingan, so long ago. It forced him to have to continue to call any multi element jutsu a KG even though they are not very different from the hidden/secret clan jutsus, but drastically differ from the clan inherited traits like eyes.
