So are you saying its all in the mind? That as long as you feel responsibility for the death you can get ms.

But it wouldn't cause him to gain MS this many years later, and Obito wasn't his best friend

No, that's all wrong. Itachi was convinced that to get the Mangekyou, you had to kill your best friend, only because he obtained it when he did. Kakashi one, has no friends to kill. All of them were KIA. You're supposed to gain the Mangekyou as soon as you kill them in that theory; why would Kakashi gain it all those years later?

Also, Sasuke did not kill his best friend. That was supposed to be Naruto, and he didn't kill him. Sasuke didn't gain the eyes right after he "killed" Naruto (Since he didn't kill him in the first place).

From what I can tell from the series, killing is not a requirement. That is just what Itachi was led to believe since he gained it from killing Shisui. So from what I can draw from the series, the requirement is different for each person.

EDIT: Also, Itachi was not his best friend. At least, until the time he killed him. You can't kill someone you hate and despise and then look back on it and say, "Oh my God. Why did I do that?", and just gain MS.

My thought on it is you have to lose somebody that is close to you. (Like Kakashi lost sasuke to Orochimaru, but until it is told to us all we can do is speculate on how he obtained MS.

I think it's like when you kill someone close to you you gain the MS, and that happens because you have to choke back your emotions and kill them. Once it's done and you realise what you did and the guilt sets in and that probably makes your body react to your mental state and then you get the MS. In Sasuke's case the guilt set in after he found out the truth about Itachi, (Although there was no choking back of emotions) and if that truth was enough to make me cry my F***ing eyes out, I'm pretty sure that was what made Sasuke get his MS and not the fact that he physically killed Itachi. In Kakashi's case I only remember him saying that he was developing that technique

Didn't Sasuke only get his cause Itachi transferred his powers to him when he poked him in the head?

Pretty much.
I also feel that it's the guilt that makes you feel that you're in complete despair to have done this atrocity. Some argues that the killing must indeed be done, but don't forget that even Itachi's case differs. The closest person was always Sasuke not Shisui. Yet he gain cause he was in complete torment just by the thought that he will have to annihilate his whole family. The simple thought of awakened his MS.

As many said, it's the guilt of losing Obito that caused Kakashi to gain MS but he took more time because he didn't kill his friend by his hands (unlike Sasuke with Itachi, Itachi with Shisui ect...) Surely, his torment went worse when Rin dies and through working to control his hate, he awakened his MS.

Even how the Rinnegan got activated with all the mental hate consumed, makes me feel that it's almost the same case for Sharingan to get into the next stage.
