So, if Illumi implanted a needle into Killua's head in order to brainwash and control him, and it wasn't removed until well into the chimera ant arc, how come when entering ngl, the needle didn't appear in the extensive medical checks, the x-ray, brain scan, the mri which would have ripped the needle out of his skull, etc.

No it definitely wasn't sooner
The needles aren't made of nen, he can just use nen with them, so it should still show up in all the tests, and be ripped out by the mri
