No it was kakashi or hiruzen. i can't remember if it were during the Land of waves arc or right before/during chuunin exam.

I am pretty sure that Hiruzen has a talk with Kakashi in the anime after he quits Anbu to become a sensei, and Hiruzen explained why he put the teams together. Other than that idk :|

They were only put on the same team because Sasuke was top of the class, and Naruto was somehow worse than background fodders. And obviously, putting your best student on the same team as a complete waste of space isn't a great way to botch up your teams and get everyone killed. Just look at how sucessful Team Minato was.
