I think Madara only used Susano'o with Rinnegan. And he probably could only do that because he already mastered his MS abilities and knew what he had while Nagato only knew Rinnegan techniques.

Probably because kakashi had a normal sharingan implanted and not MS. So when he was close to Obito and his eye's transformed into MS kakashi's did to since they have the same eye.

Kakashi was a special circumstance. He was within proximity of Obito when Obito underwent the change from Sharingan to Mangenkyo. So His body probably retained the muscle memory of shifting bwetween Sharingan and MS. Particularly after Itachi used it on him back in Part one.

there was a thread about this a long time ago when that 1st happened. Everyone raged about it and it was kind of a conclusion that the awakening from Sharingan to Rinnegan repaired (restored) his lost eye......
