Gai is faster in base for sure but they both have a 5 in taijustu skill. Tsunade can most definitely take a base Gai attack while Gai cant take a Base Tsunade attack. Basically no way Gai dodges everything especially when they are fighting right in front of each other.

Also no kunai will be cutting Tsunade's head off, she is slightly less durable than Ay and would you say a Kunai cuts Ay's head off? Plus Orochimaru

Gai isnt sticking a kunai into her skull before Tsunade simply puts her hands up to defend herself.

No, she's not. With whatever non-manga and databook supported reasoning is beyond me, and I suppose the author of this manga himself as well.

She's not durable enough to withstand the output of damage he can inflict on her in base form. Especially without healing.

She's still been shown to be impaled by weaponry/jutsu. Durable yes, but still human flesh. Gai would do what Lee did vs Gaara find an opening, then Kunai to that big ass forehead.
