No Itachi could've slit Kakashi's throat in the Genjutsu. The second time Itachi used tsukiyomi on Kakashi's clone he tried to burn him like paper.

Genjutsu is capable of killing. You can mind fuq somebody until eventually it's to much for them to handle. Kakashi himself was surprised Itachi didn't kill him in tsyukomi. This implies it can kill him because genjutsu can do that to someone. He would not have said it otherwise.
