In Asian traddition, only a male leader can create a new clan or lead the old one. Otherword, only men can hold power of a family, not a woman.
However, Sasuke and Sakura only yielded a daughter. If he does not continue to breed a son, the Uchiha clan will certainly go to the abyss of extinction. In the tradditional tone of course. If he breeds with Karin, however, may be a super Uchiha with super Uzumaki stamina??
