Yamamoto said that no one born in the last 1000 years has surpassed him. Some, if not all, the RG are much older then 1000. The fact that Yamamoto said "in the last 1000 years" and not "ever" implies that there where people born before 1000 years that where stronger then him. Ōetsu and Ichibei are two of those people.

He said in the last 1000 years, because he hold position of CC for so long, actually he is CC for 1000 years. That doesn't mean that there exist shinigami who is more powerful than him outside of 1000 years.

Yea Yama himself is not physicaly the strongest shinigami but with his zan added he becomes the Shinigami with the greatest attack power then next comes zaraki,that's untill these royal guards show more.
