Before you start ranting, i'm not saying it to degrade him. This isn't my work.
Let me explain. Throughout the whole story we've seen Naruto try and fail to change Sasuke. We've seen that each and every time he tried Sasuke was ahead of him and out of reach. This panel, I believe, plays into that whole part of the story, it symbolizes that Naruto is more determined than ever to finally change Sasuke, but at the same time he is nervous about actually fighting him dead on. He knows how stubborn Sasuke is, hence the long sigh, but is still willing to give it his all. Think of it like the incredible Hulk, when he's losing to the Abomination, he sighs and looks down as the Abomination taunts him, but refuses to give up. That's what I think Kishi is trying to show. Naruto knows just how powerful Sasuke is and is intimidated by him, but is willing to do anything to finally change Sasuke and the world.
