Tsunade could heal the physical affliction that was a side effect of the seal. But she could never put the soul back in his arms.

This is shown when Orochimaru switches bodies. He may have freed himself from the body that was deterioating but his arms still couldn't do jutsu till he sliced the death gods stomach open.

And I said she could've pulled the arms away from the reaper. She is strong, she can touch spiritual chakra, as seen when she helped pull the tailed beast chakra,
