It's pretty much an exaggerated form of Tsunade's eye lol.

And her seal is release, I was going to draw Hahsirama circle on her forehead, but in made it her eyes instead.

You realize, that in order to summon katsuya, you must have the "Strength of a Hundred Seals." Basically, the seal on Tsuande's forehead, which amplifies her abilities. Pehaps this, is the form of sage mode you are referring to. Maybe I'm wrong though, as sage energy is nature energy, not energy created in one's body.

The seal is directly connected to Katsuyu, and the forest,
Katsuyu gets her power from Sakura's chakra, who's to say they can't reverse the connection, and let Katsuyu give natural energy to Sakura.
