Sasuke - Anbu Leader, Possible Hokage. (less likely than Naruto)
Sakura - Jonin. Becomes a medical ninja even better than Tsunade.
Sai - Possible Anbu Leader, if not an elite Anbu.
Lee - Special Jonin of taijutsu.
Tenten - Special Jonin of ninja tools.
Shikamaru - Hokage advisor.
Ino - Special Jonin of interrogation.
Choji - Elite Jonin, Leader of Akimichi's
Shino - Elite Anbu.
Kiba - Anbu tracker, or Special Jonin of tracking.
Hinata - Jonin, Possible Hyuuga head, surpasses Neji eventually.

Tsunade gave Naruto the necklace and the 'kiss' so she wanted him to be Hokage.
She will give up the Hokage seat for Naruto, so her death is not a factor.
