The ones that bother me the most are:

Madera using susanoo without having eyes.

Madera using kamui as the jubi jin, when obito wasn't able to when he was jubi jin.

id like to help you answer it.

1. madara using susanoo
i believe the ems was just the trigger to using the susanoo. once you've unlocked it, even without ems you still can use susanoo. think about this, when he was still edo and using RINNEGAN he could use susanoo. in fact susanoo is the doujutsu of ems, not rinnegan. that means, eye just the trigger.

2. madara using kamui
IMO its not like he wasnt able, but it would got heavier to jump using kamui if you're juubi jin. he could still do it, but it was harder than if you're not juubi jin

tobirama died sacrificing himself for the youngsters (hiruzen danzo and other ninjas)
hiruzen died after sealing oro hands when he was old
minato died young sealing kyuubi inside naruto

Not really a asspull, but obito pulling out a piece of the one tail and eight tails chakra. It just "happened" to be the only chakra's naruto was missing
