Sickness holds him back. However, Itachi does have a cool head like you pointed out. I see your argument, but I think Madara has higher chakra reserves than Itachi. The fact Itachi was so cautious, not wasting his movements, means he likely didn't have huge chakra levels like Madara.

It's something I've been noticing throughout the series. The more intelligent characters tend to have handicaps. Their intelligence and high observation is balanced out by lacking something, otherwise they'd be god-tier.

In recent chapters we learn that Madara hand seal speed is quite fast to where he can form seals mid combat. We also learn that he is always calm and collected and has plans on top of plans.
We further see that he likes to mix multiple styles including Tai + gen and Nin + tai. Finally he is quite knowledgeable and quick to act and think.

With all of this information we can honestly say him and Itachi are splitting images of one another except for the fact that Itachi does not have killing intent.

Meaning... When Madara goes all out. However his combat style will be we can safely assume Itachi would be the same way.

I would agree to that as far as Itachi and Madara both being extremely intellegent and collected in battles.
