One major death (see: Neji) in the fourth Ninja World War is obviously not enough. Shikaku and the others with him were also tragic, but less significant. It seemed that surely, Tsunade would die while healing the other kage. This would have served the purpose of sacrificing herself to keep them alive while also re-opening the position of hokage.
Incidentally, Kishimoto decided to have all the kage, including Tsunade, survive - as seen in today's chapter.
Her survival, combined with the final panel of the chapter, makes me realize something. Yeah, Kishimoto does recognize that there needs to be at least one more major death for this war. He isn't skipping over it.
He's just decided to make it Kakashi instead of Tsunade.
Not to say that the attack in the final panel will kill him, no - I suspect it'll be chalked up to illusion, or he'll be healed, or something of the sort.
But in the end, we can see, almost undeniably*, now, that Kishimoto intends to have Kakashi killed - probably along with Obito.
