You are thinking way too much into it.
If the current Hokage elects you, it's decided. Tobirama did it to Hiruzen. These other "hurdles" are only tere when there is no Hokage. Even though the village apparently wanted Orochimaru, Minato was chosen by Hiruzen.
If Naruto declined and demands that Tsunade make Sasuke Hokage, she has very little choice but to do so.
Isn't it possible that the Hokage could choose his successor and that maybe others (feudal lord, jonins, council) have to approve of the decision? We didn't really see the whole process in the manga so it's difficult to assume one way or the other.
Regardless, a good Hokage wouldn't ignore the advice and wishes of everyone else when choosing a successor. They're all affected by the decision too.
