I don't think he's fanbased. I believe that he was either mentioned during the Itachi vs Sasuke fight or the Madara and Sasuke conversation. They talked about how Madara stole his eyes after he went blind and that's how he got the EMS because of how Madara's sharingan was and how Izuna's sharingan was and with them combined he made a stronger version of the MS. If Izuna was fanbased then Madara wouldn't have the EMS. He would have went blind a long time ago.
