In terms of openings, probably Steins;Gate's, Fate/Zero's 2nd one, Hunter x Hunter's. And maybe SAO II's 2nd opening, Shigatsu's 1st, Black Bullet's, No Game No Life's...I could go on and on <_>

Endings wise, I don't actually watch a lot of them, but if I had to say one, HxH's 5th one.
