Obito knew that Itachi has Shisui's eye => KA. Obito knew that KA is a one hit shot and also knew that Itachi will use it only on those who pose a big threat such as he would be. So, no, Obito was not afraid of Itachi. He was afraid of KA and knew from the start that if he attacks Konoha he is the only MAJOR THREAT TO THE SHINOBI WORLD because Madara was dead. Ka scared Obito. Because he was the only viable threat to be used on, if he decide to attack Konoha and later on the whole shinobi world. The other major threats such as: Madara were dead and Sasuke was between good and evil. Itachi planned to use KA on Sasuke through Naruto.

Itachi programmed the crow to use KA on Sasuke if Naruto was in danger. The only way for Naruto to be in danger is if Sasuke decided to attack him therefor killing the only person that wants to be friends with Sasuke since he was considered a traitor at that moment.

The moment when Itachi used KA on Nagato, Nagato didn't have Itachi's eyes. If i'm not mistaking he had double rinnegan. I don't remember well what eyes he had. So the crow affected another eye not his own.

The important fact is that you are 50% right. The other 50% is that the programming didn't restrict KA on one person. It just made it it's priority to attack Sasuke if something happened. If let's say Madara + Sasuke vs Naruto, even if Madara would pose the biggest threat, the crow would prioritise and target Sasuke.
