Amaterasu is can shot directly by the sharingan or indirectly in enton form.If a uchiha target someone with their sharingan they can be burn by wherever amaterasu spawn.Why Itachi or Sasuke use their sharingan to view their oponent internally and spawn the amaterasu at the target they desire in the oponent body

It spawns where the user is focused.

The reason they cannot do what you are asking is because that wasn't a picture of what Sasuke was looking at. Sasuke could only see them going into his body. The picture was shown for the readers to see what was happening inside Sasuke's body.
