Tsunade created the medical ninja system, which makes her as talented as the previous Hokage.

Quite a lot better, since the ninja system is failing horribly, the Konoha police is all but eradicated, Hiruzen and Minato never set up anything.

Oh, and Tsunade > Hiruzen. On the political side of being a kage, it's not even a contest. On getting fodderized, Kabuto took more than one chapter to beat her while Hiruzen got k.o'd in one page by an academy student.
