The same reason why Tsunade looks young despite being old.

Senju have strong life forces and vitality. Hashirama already had tsuandes healing and regeneration. It's not surprising that he could keep his youthful body.

Well Hashirama body seems to be the epitome of perfection, so I agree. However the timeline in Naruto seems weird. Everything after the establishment of the leaf village must have be done within 150 years or so. I don't believe it was past 200 years. Then again Kakuzo attempted to kill Hashirama, so Hashirama could have been 80-90 ish then.
