and it suits the characters that have it (almost has a meaning behind it) :

Itachi : the man who did the unthinkable and cried tears of blood.

Sasuke : the kid who lost it all, was tricked to killing his own brother for protecting him and soon will be the last of his kind/clan (a lonely Uchiha).

Depends on how much strain he puts, the higher the concentration the thicker the flames the thicker the flames the more his eye bleeds.

as in one panel he didn't bleed using it but in the ice world he did notice how much thicker the flames are in the ice world then the lava world so its just a letter of strain pressure.
