maybe initially toad sage mode was the only sage mode existed , so naturally turning into a frog would suit the toad theme..

The way I look at it was kabuto was not a perfect sage hence being all scaley. And I'm guessing he turned snake and not toad was because each creature has a different part of natural energy they draw in.

Natural energy is natural energy, it comes from the world, learning it from the frogs, snakes, or slugs should have no aesthetic changes.
