Sasuke has EMS which is a lot stronger than a Curse Mark. The pain on his body from using the MS is reduced and there is no strain on his eyes. He also has complete Susano'o and he has mastered enton.

We all know the fight will come down to Susanoo where Sasuke as we all know has the superior stamina and eyes to keep his Susanoo far longer than Itachi and if Itachi tries to use Amaterasu....not only will he be wasting Chakra, but also giving the advantage to Sasuke since he can manipulate Amaterasu any shape or form he wants and use it against Itachi.

Sasuke easily- he counters all of itachi's genjutsu inc tsukuyomi, is superior at amaterasu, his stamina is on a completely different level, his katon is stronger, he has a and s-rank lightning techniques, and he can hold susanoo for longer. unless the totsuka sword can hit a fast moving target or pierce through susanoo itachi doesn't have a shot and he can't use full susanoo for long.
