Yeah I'd say Tsunade should take the win. Tsunade's regeneration is too much for most of Guy's repertoire. Guy needs to use the gates from the very beginning of the match to get a chance at victory and that seems improbable. Not to mention Tsunade has Katsuyu on her side to assist in battle or in healing.

The same could be said of Guy's gates, they'll eventually destroy his body, if Guy really wants to hurt her he'd need to use the 7th gate and by then he's fatigued himself.

I just can't imagine that base Guy is too fast for Tsunade, Tsunade may not be a speedster but a Kage level ninja should at the very least have the reaction speed to keep up with base Jonin speed, even if Guy is exceptional in this area. Regardless of speed anything that Guy can use to actually do good damage to Tsunade will exhaust him and if Guy fails to take Tsunade out with gates, he's screwed with the aftereffects.
