Naruto's not as ruthless as Itachi. Besides we've seen more foreshadowing about Sasuke killing Naruto, not the other way around.

And the biggest foreshadowing I think this series has ever had is when Naruto tells Sasuke "We're both gonna die." at their 2nd Shippuden fight.
