If anything he got a transplant or an ORO experiment gone wrong. It looks like he cant turn it off, just like kakashi. Thats why he covers it.

It's possible that they're two completely different people.

If you think about it by Age:

Madara should be much older than Danzo, yet how was Danzo able be the same age as The Third's Advisors and the Third Hokage himself?...

Think about it, unless Madara is an expert Medic-Nin like Tsunade, he may not have that sort of Forbidden-Medical Jutsu to appear younger.
