It's tough to see how it could be Madara. He assumedly had to stay low after his defeat. I can buy he can hide his chakra, but not that he can hide it and fight Hashi to the death at the same time. Sensors (like Tobirama, who outlived Hashi) would have been like "hey, what's that big chakra near where my brother is? Oh, it's Madara... typical". The guy can currently sense Madara at a range of many, many miles as it is.

Kurama+mandara fight hashirama, and mandara lose control to kurama then hashirama sealed to his wife, this is why the first person who have kurama is the wife of hashirama then pass to kushina then to naruto. Who knows but i believe for sure that he never died in a battle
Maybe here my theory
Madara said He had a regenration jutsu with no hand seals like Tsunade
And we all know that the side effects of Tsunades jutsu is that it shorten the life span of the user because they create new cells
And maybe hashirama used that jutsu so often that he has shorten his and died of the side effects
It is just a theory it can change any time in the future chapters
But he woud never die in a battle
