I also think it was old age, and about the revival of how he is now. lets not forget what kabuto said, that he made madara's summoning a bit different. do the words "you have been completed beyond what you were in your prime" meen anything??
I belive this is supposed to be interpreted as giving back madara's body as it was befor the fight with the first (wich left him seriously dammaged, a shell of his former self) but it included any advances he made from that point, his EMS transforming into Rinnegan and all the powers that came with it. perhaps his chakra reserves were cut down sevearly by the first and he was unable to actualy use the rinnegan propperly, but now he is.

Soo... i just had an ephiphany:

Madara probably killed himself.

We know from chapters 560 and 561 that Madara gained a portion of Harashima's power, thus allowing him to advance his Eternal Mangeyko Sharingan to the Rinnegan.

We also know, both from the manga and the anime, that he was severely weakened at that point.

Here is where we get into speculation.

Again, from chapter 560 we know that Madara's fusion with the Senju DNA was flawed. The implication is that if the fusion was perfect, there'd be no need for Kabuto to augment and perfect Madara's body.

Now, what happens when someone pushed to their limit tries to use a Sharingan and senju-chakra power?

we have Tobi, posing as Madara, stating that this is the mass of cells that he stole from Harashima.

Madara used his Rinnegan to do something and the new Harashima DNA went crazy. The result produced the White Zetsu.

Who ever says it was the first hokage is an idiot.

Madara survived the fight, obtained his DNA and lived longer. Met with child Nagato and everything about it is true. Madara knows of IWR and why it was used, i highly doubt that 2nd Hokage used it during Madaras time. Probably after and found out while he was in the mist. Madara probably died due to sickness or some experiments, he and zetsu were doing. Personally think Tobi is Madara, unless proven otherwise and will continue to be. Something of Muu's split ability.

Edo Madara straight out stated that Impure World Ressurection was an Uchiha jutsu, developed so that they could summon the asteroid things to wipe the battlefield clear.

That means the second Hokage learned the technique from them.

I believe that's in Chapter 560 as well. I've spent a bit of time looking at that fight in the last little bit.

We don't know if Madara was killed and have little data even for speculations about his death.

The police force of Konoha didn't investigate mysterious deaths in their village- be it Madara or Minato. They didn't even pay attention to the ANBU and Biwako's deaths whose death should have arisen suspecions as they didn't died by Kyuubi's attack..
