if you mean without any enhancements like opening of gates, chakra etc.

then I'd say Minato, Tobirama, shisui (he was known for his shunshin apparently)

Without any enhancements, these characters have the fastest reaction-rates and foot-speeds as well:

In no particular order:

- Itachi, Sasuke - Kenjutsu proves this point as well - , Obito, and Madara from Uchiha.
- Killer Bee - Kenjutsu being the main point as well.
- Minato
- Gai and Lee, no doubt.
- Tobirama has pretty good reaction rates as well.

That's just about it, I think.
