Itachi formed a Shadow Clone while in a shuriken fight with Sasuke and Sasuke was shocked also in part 1 Kurenai was extremely shocked/surprised at how fast he could form his hand signs

Kakashi is pretty fast as well when it comes to forming hand signs in numerous battles

Kabuto with Orochimaru's cells and stolen power formed hand signs extremly fast as well against the Uchiha Bros
