itachi had the worst internal pain, because nobody knew about it and he could'nt talk to anyone about it.

madara - didn't get his way

kakashi - pretty much the reason his entire team is dead

I gotta go with Itachi, though Sasuke is pretty close.

It's between Sasuke, Obito, and Kakashi.

Sasuke: whole family killed at young age, killed his own brother, realized brother wasn't evil, cut off ties with his friends, felt like everyone was mocking him (went psycho.)
Obito: friend betrayed him, Rin died, spent half of his life in a cave with a weirdo (no offense Madara), worked on a plan for 18 years that is going to fail because of plot no jutsu.
Kakashi: believed Obito died for him and betrayed his *dying* wish by killing Rin (he was guilty about that situation for 18+years), sensei died and he wasn't allowed to help fight, student became evil and tried to kill him, dead friend returned after 18 years to kill him and take over the world....

I think Obito takes the cake though. Here's why: he wasn't really happy during those 18 years.

Wow, lots of threads of these aren't there? Personally, the one with the most pain to me was Nagato, but I start to realize now that he did die knowing his legacy could be passed on and I suppose you can say died "peacefully" I guess the one with alot of pain at this current time would have to be either Naruto or Sasuke. Sasuke for matters in which he is still constantly seeking answers, thus I doubt he is satisfied with where he is currently standing. Naruto well, as you know he is fighting for the fate of his world, trying to get his friend back, AND help save everyone he loves so to me, that's alot of stress and pain for one young man to carry. Not to mention both of their pasts and all. So yeah, them too lol

Itachi- Had to slaughter the few for the happiness of the many, that few was his entire family. And he couldnt explain to anyone why.
Sasuke- Having everyone you love and care about slaughtered and theres nothing u can do to stop it.
Nagato- Having your parents killed right in front of you, then being forced to kill your best friend.
Gaara- Having everyone hate you for being alive, the pain never going away, being suicidal with no way to kill yourself cuz the thing inside you wouldnt let u die
obito - Watching the love of your life killed right in front of you, unable to do anything about it.
Tsunade- The love of her life, and her lil brother killed
Kakashi- Everyone he has ever cared about was killed
Kabuto- Orphaned and never understanding why and who he was.
Naruto- No friends, no family
Madara- Being second to Hashirama
