Great topic. It's an incredibly difficult choice. But I'm going with Gaara, considering the things he did. Kakashi, Tsunade and Jiraiya has their regrets as well, but they weren't complete psychopaths. They were just unfortunate when it came to certain things.

Sakumo: Atleast do not kill yourself in the living room
Hiashi: Failed to protect his brother, and his nephew; if one of my siblings were to do that they would be doomed in their eternal life
Gai and/or Lee: They did not show a sign of regret, but i am sure they are regretful about failing to protect Neji
