Kabuto said he didn't revive Shisui because he couldn't find his body anywhere. Itachi didn't kill Shisui, he committed suicide. To get the mangekyo you must not neccesarily kill your best friend, you must only see him die, and killing him is the easier way to see him die, rather than waiting.

This would answer who Itachi got his MS that just leaves shisui?

Itachi got Mangekyou when Sasuke was about 6. Naruto is now 10 years old and they have the same age. Therefore Shisui could have used genjutsu in Itachi, then 10 years later, Naruto was able to use it. Itachi said he couldn't use in sasuke because he couldn't, and probably because it wasn't activated yet

I already made this thread today and it is quite clear by the manga scan left on the second page that he is dead, but we have no idea how or where..

There are several translations of the manga from Japanese to english, same concerns with the anime and is why dialogue is often different in comparing the chapters to episodes.

Itachi had make sure nobody could found Shisui body.
In Naruto world, information from either dead or alive ninja could be extracted.
Danzo got his eye.
He gave the other eye to Itachi.
Uchiha Shisui was the only one that could reached the highest level of MS.

Anime and Manga are by different people.
Manga is from Kishi himself.
Anime is done by TV with Kishi approval. Just to ensure the characters not change....
