Captain Yamato was captured in chapter 514 by kabuto so madara could use hashirama's DNA to strengthen the white zetsu, and its been a long time since we've seen him.. is he ever coming back or did Kishimoto forget about him??

He was in SZ and now he is in IT.

If you remember, the SA was fighting with spiral zetsu when Naruto/Sasuke were fighting with Madara. Yamato was inside the spiral zetsu being used to amplify spiral's powers. When IT started, spiral zetsu didn't need yamato's body anymore so he separated from him (chapter 677):
