What does it mean when a human is in the palm of complete and utter despair?
When there is no hope .
It's a point where everything is lost and you know it. But more so, the event or circumstance isn't over and you know it. Thus, meaning that you can see everything or anything you can attempt cannot alter an event. And naturally, these all imply a negative connotation, otherwise there's no need for despair.
Examples:
You have an exam in one hour that decides your future, and you KNOW that you don't know a single answer you answer will be fight.
Or
Let's say you are part of an army defending your home. The enemy far out numbers you, and you know you will die. Just before they attack, they reveal the heads of every family and friend of your army. Meaning you're now going to die for nothing. The ones you sought to save have been murdered as you soon will be.
It's being in an inescapable negative situation, and knowing it.

Well, I think that each person has a different concept of "despair"
To me, despair means losing the people I love.
