Kishi always finds a way to restrict otherwise op ninja's however i believe its the ms he activated from a young age especially the side effects of susano.

Well in this case Sasuke should also have a disease, no? The reason I say this is because Itachi used his MS sparingly while he was alive. He was already skilled without it but sometimes for plot purposes Itachi used MS when IMO it wasn't even needed.

I'm going with pneumonia because he was coughing up blood.
