What killed Itachi?

I know Itachi died from a sickness/disease/infection. But what kind was it? And how could've Itachi have obtain it? I don't remember if they metioned any of this in the manga/anime, but if they did, someoe please show me a scan or link. And if they didn't, someone post a well explainef theory as to maybe what the sickness may have been and as to how.
