Hashirama felt guilty after killing Madara. He stepped down from being Hokage shortly after. He started to realize that Madara is right about family and friends come first before the village. So he spent some time with his granddaughter. Then he kill himself or die to his healing ability. The body can only heal so much.

Tobirama became Hokage very shortly after Hashirama so it seems unlikely he would have died of old age. My theory is that he exhausted his life force from the overuse of mokuton in his fight with Madara and either died immediately after or shortly after.

For someone that can manipulate her appearance with chakra to look 30 years younger, a little breast enhancing jutsu should be relatively easy.
