I think it has to be Obito conveniently activating his Space-Time jutsu for the first time and "falling" through the ground to a secret layer that Madara just happened to have sitting below that particular cave. Out of all the places in the world to have a hide out, Madara picks a cave in the middle of a war zone, a very discrete location. Also the jutsu magically knows that the soil above the cave has ended and the phasing needs to stop when Obito hits the floor of the cave. Lets not forget that the jutsu has a time limit of 5 minuets. If all 5 minuets of the jutsu are used then, falling at the rate of gravity:

Distance = 32.2 ft/s^2 * (5*60s) = 9660 ft or 1.829 miles.

I know that cave may be deep underground, but that is REALLY deep.
