Unlike Bleach, Oda so far hasn't used surnames as a way to call someone, however typically you do say a Doctors last name rather than his first, the exception to this which possibly Oda might be sticking to is Chopper, people have called him Dr Chopper, if his first name is that from Tony Tony Chopper. However, I'm fairly certain someone has called Luffy "Monkey", depending on how you take the context they could either be referring to him as being like a monkey (probably this), or calling him by his surname.
